At first glance some of the questions might seem a little odd and delve into personal information you may not wish to divulge. However these questions are asked for a reason and all are relevant to the outcome of the final product.

Please Print Out this page, and on additional sheets of paper begin to answer these questions. We understand that many of these questions may take time to answer so please take your time with each of them and be as thorough as you can. Ask questions with family members, friends of the family, etc.

6. Where did all of the above graduate College and in what years?

7. What career did/does all of the above work in?

8. If any of the above were divorced, in what year and where?

9. What military experience did any of the above have?

10. What information from the above questions can you answer about your siblings?

11. Do you know why you, your spouse, siblings, or children were given the names they have?

12. What do you remember about your childhood?

13. Where did you, your spouse or children go to Elementary School and Junior High?

14. Where and when were your parents, grandparents, and great-grandparents born?

28. Do you know who in your family originally immigrated to this country?  Where did they come from?  Why did they leave their native country?

29. Were there any diseases or medical conditions that seemed to be prevalent throughout your family history?
